I am aware my claim will not be processed until I forward the following, and failure to do so within 28 days will render my claim null and void:

1. Statement of claim against Bullet Express.

2. Cost Invoice for the entire consignment. This must be for the purchase price of the goods showing any discounts and rebates. For produced goods estimated cost of production.

3. Sales invoice to your customer.
